What sorts of things make the money exchange rates in banks go up and down?

How is the value of money, exchange rates, etc. decided?
When we hear "the dollar is up on the pound by x amount" how does this happen? Why can't countries just manufacture more money? This is a naive question perhaps, especially from a 30 year old. But I am interested to know how yet have never found anyone who can give me a definitive answer. Thanks Hypnotic OK this is really helpful. But how are the governments aware of how much is exported / imported? If a private business in the UK does business with a business in America, then how does that affect the weight of the pound vs the dollar, or other currencies? Or am i getting this wrong?
